Important Reminders

Please bring information to complete a Patient Registration Form and Health Report. Including having an Emergency contact for our office.
Payment Arrangements – We accept Cash, Check, Visa, Master Card and Discover. At your convenience, there is also Care Credit finance company available (but can NOT offer deal discounts with this form of payment).
We maintain our 24 hr, 7 days a week phone service, in addition to our normal office phone hours (7 a.m. – 7 p.m. Monday through Sunday), our staff can answer your calls and connect you with the appropriate information and expert service provider. Even if it means the doctor returning your calls, after office hours.
You may receive a courtesy confirmation call a few days before your appointment. If you’re unable to make it, please cancel ASAP, be sure to give at least a 24 hours beforehand or fees may apply for repeat No-Show offenders.

NOTE:

We have a working schedule, so we can keep your wait to the minimum, you may get a call or text inquiring if you’d like to move your appointment slightly earlier or later to reduce wait time for all patients throughout the day.
